Overview
Our Life Stage Transitions service helps you plan for and settle into big changes, like leaving school, moving out of home, starting work or adjusting after a change in health, with practical support and skill building along the way.
This Capacity Building support is designed for participants who require guidance when moving into new stages of life, such as starting school, finding employment, changing living arrangements, or preparing for greater independence.
Support may include transition planning, mentoring, help with decision making, daily planning and budgeting, and building the skills and confidence you need for the next stage of life.
Each plan is tailored to individual circumstances, strengths, and long-term aspirations.
We collaborate with families, support coordinators, and allied professionals to create consistent pathways toward stability and growth.
What we help with
We help you understand your NDIS plan, your funding and your options, so you can make informed choices about your supports.
We help you get ready for the change, practise new routines and connect with community groups and mainstream services. If you have a support coordinator, we work alongside them.
Together we set clear goals for your next stage of life, such as starting school, finding work or moving home, and plan the supports needed to reach them.
We help you plan ahead for difficult situations, so you and your family know what to do and who to contact if circumstances change suddenly.
